Claim:
During a police chase, I got out of the vehicle and ran straight to a ladder and started to climb it. While I was at the top portion of the ladder, which was at least 20-25 ft off of the ground, Officer Phoenix badge C2-15 decided to taze me in the back. Due to being tazed, I let go of the ladder and fell to the ground landing on my head and neck. The fall caused me to lose total feeling in my legs from the hips down. Also, I was experiencing severe pain in my neck and in my back. I was then handcuffed and taken to a jail cell. While in the cell, I requested to be seen by a doctor and/or EMS with multiple LSPD officers in the room. I was denied treatment and told that Bolingbrook prison had adequate medical staff to treat me. When I arrived at the prison, they did not have any medical staff available to treat me. DOC had to request EMS to come to the prison. EMS evaluated me and gave me a couple medications, and I was told to reach out if my condition got worse. Later that evening while I was laying in my jail cell; my condition was getting worse. I tried to get up out of bed to go to DOC staff and that is when I fell out of bed and could not move my legs or get up off of the ground. DOC and EMS came to my cell and transported me to Pillbox Hospital. At the hospital the doctors were able to give me a better evaluation and take an MRI and X-RAY. The tests showed that I have multiple bulging discs that are on my spinal cord causing me to lose feeling in my legs and other injuries that are not as severe. I have an upcoming surgery to try and regain feeling back in my legs. The doctors do not know if or when I will regain feeling and movement. I was informed if surgery goes well, it will be a long recovery with months of physical therapy, or I may never be able to properly use my legs again.

Demand:
Compensation for poor quality of life, medical bills, loss of wages due to not being able to work. Also, I want the officer and the LSPD to take accountability of their actions, even if that means filing criminal charges against that officer.

Witnesses:
Officer C2-15 and the other officers working at LSPD at the time of incident. DOC and EMS are witnesses of my medical conditions.

Evidence: Any and all evidence will be provided through my lawyer.
